This study mapped the development of insight in the 4 years after presentation with first-episode schizophrenia & schizophreniform disorder & determined the effects of evolving insight on depression & the likelihood of attempted suicide. 101 individuals were assessed at presentation, 6 months, & 4 years. Insight, including recognition of mental illness, recognition of need for treatment, & the ability to relabel psychotic symptoms, was measured. Depression was measured & all suicide attempts were recorded. Insight improved with time. Recognition of mental illness at 6 months predicted depression & attempted suicide at 4 years. Findings may have implications for the disclosure of diagnosis. (33 refs.)
