The case of a 58 yr old woman is used to discuss issues involved in developing an orientation toward the treatment of very ill psychiatric patients. Issues include the question of who needs what type of care, decisions about investment of resources, treatment objectives, assessment of the success or failure of treatment efforts & forms of co-operation in the treatment of these patients. Theory & practice need to be measured against the extremely complicated clinical realities of such cases.
