The author argues in favour of broadening the scope of risk management in child & adolescent health services to include not only self-harm, harm done to others, & harm done by others, but also harm incurred by the system itself, & harm done to mental health staff. Recent policy developments for the reporting & monitoring of risk management services are reviewed, including external inspections, external inquiry systems, serious case reviews, other external reporting, & internal inquiries.
