The reporting pathways & statistics between relevant stakeholders for reporting suicides & undetermined deaths in Cornwall were evaluated. The correlation of the numbers provided for suicides & open verdicts given by different national & regional agencies were checked. Results show the data transfer between the relevant agencies can be inaccurate. These inaccuracies not only prevent establishment of clear records but also could impact on policy decisions. It is important to recognize the National Health Service trusts involved in mental health are being evaluated on their record on suicide prevention, thus making it important for these differing figures from different agencies be taken into account by inspection bodies.
