This document provides detailed & practical guidance to developing clinical audit of suicides within primary care trust localities. It describes national policy guidance, explores the concept of population suicide audit & its relation to clinical suicide audit, & describes the routine & non-routine sources of data. It also reviews existing literature to determine methodologies that have been used for suicide audit & identify those which are more effective in improving clinical practice as well describing the difference between adverse incident reviews & clinical audit of suicides.
