This report examines the impact of medical errors in psychiatry & identifies specific areas for improving patient safety, including suicide prevention. Broad recommendations for the American Psychiatric Association to engage psychiatrists in a national effort to improve patient safety in psychiatric care are presented, along with a second level of specific recommendations for those in a position to act. The process & deliberations of the Task Force that led to its conclusions & recommendations are also described.
