The process of an inquiry in the UK into the deaths of mentally ill persons through homicide or suicide is described. The purpose of this inquiry is to provide information so that changes in clinical management to reduce the numbers of such deaths might occur. One aspect of this inquiry is surveying psychiatrists who have worked with mentally-ill patients before their death, and asking them that with hindsight, if they think the death could have been prevented.
