Among patients with schizophrenia, 40% report suicidal thoughts, 20-40% make suicide attempts, & 9-13% complete suicide. Clozapine is the only antipsychotic drug that has been shown in controlled trials to be effective in reducing both positive & negative symptoms in patients who fail to respond to typical neuroleptic drugs. The potential decrease in suicide among patients treated with clozapine is estimated to be as high as 85% & thus outweighs the low risk of mortality from agranulocytosis.
