The rapid growth of managed care has accelerated the evolution of the clinical record. This article discusses the documentation of accountability in the case of potential client suicide or violence towards others. In the case of suicide, documentation should include explicit assessment of sucide risk at all levels of care, details of a no-suicide contract (if one has been used), factors considered in deciding on the intervention, & records of previous treatment.
