This article discusses the aftermath of the suicide of a 23-year-old female patient who had been in a mental health unit for 4 weeks. Details about this patient’s life are provided. The review which followed her suicide is discussed. A framework for such a review is provided, & incorporates the following issues: identification of vulnerability & assessment, management plans, communication & interpreters, discharge planning & continuing care, & counselling for relatives, patients & staff.
