In this letter to the editor, the authors comment on a previous article by Boer & Briscoe in which they stated that their analysis of overall rates of suicide did not reveal evidence that a change of emphasis towards community care is associated with demonstrable effects on suicide rates. The present authors suggest that a more sensitive measure of the effectiveness of a mental-health service is the rate of suicide among people who have made direct contact with the service.
