Musical hallucinations, whether arising as release phenomena or focal irritation, are basically similar. Focally triggered hallucinations are more liable to appear in external space in the first instance until the individual gains insight into their nature. In other circumstances, musical hallucinations seem to come from the ears or head. The music often appears pleasurable at first, but later can become insistent & all pervading until a psychotic or even suicidal state is reached. (38 refs.)
