Reports a case that underscores the need for careful screening of patients with organophosphate intoxication before transfer to psychiatric facilities. The patient described demonstrated a recently defined clinical syndrome that required emergency medical & psychiatric management. Organophosphate intoxication is a common method of attempted suicide, which may be superimposed on other illnesses such as alcoholism. Anticipation of the “intermediate syndrome” should decrease lethality. (12 ref)
