PATCH (Planned Approach to Community Health) provides a forum through which community health workers, health education professionals and citizens plan, conduct and evaluate health promotion programs at the community level. There are 5 distinct yet interrelated phases to the PATCH process: Skill in community organization, data collection and anaysis, problem specification and priority setting, intervention planning and implementation, and program evaluation.
