Maprotiline, a tetracyclic antidepressant, has a pattern of toxicity different from tricyclics. Overdosage appears more likely to cause seizures but less likely to cause the peripheral autonomic & cardiac manifestations seen with the tricyclics. Two cases of Maprotiline overdose, complicated by seizures despite few signs of anticholinergic or cardiotoxic effects, are presented. The authors stress cautious use of physostigmine salicylate in maprotiline because it may cause convulsions. (NBB)
