Self-evaluation after a suicide on the ward highlighted a fundamental dilemma between the interpersonal relationship philosophy & restrictive suicide precautions. 4 possible “failure factors” were recognised. A new suicide precaution approach, considered consonant with ward philsophy, was adopted, emphasizing a “one-to-one relationship” & “attend at all times”.
